We are a Patient-Centered Medical Home (PCMH) which is a health care setting that provides care that is respectful of, and responsive to individual patient preferences, needs and values, and ensures that patient values guide all clinic decisions.

Family Medicine providers care for the health of every member of the family regardless of age, gender or medical condition. They serve as advocates for every aspect of your health, accessing health consultants, community resources and other services you may need. Each Southwestern Medical Clinic family physician is board certified by the American Board of Family Practice.

Internal Medicine physicians and providers are here to see patients who may have a more complicated medical history.  They see patients over the age of 18 regardless of gender or medical condition.  Each Internal Medicine physician is board certified by the American Board of Internal Medicine.

Our Care Providers welcome you to call for an appointment.

We also provide the following services: Laboratory Services are offered at our location. Radiology or X-ray, Mammography and Bone Density services are offered at our location. Care After Hours - A physician is on call seven days a week, 24 hours a day.  Call your clinic's primary number and the answering service will take a message and contact the on-call physician. Medical Advice - The nursing staff is available to help with any medical need, test results, advice or concerns you may have. Medication Refills - Please allow 24 to 72 hours for your prescription to be processed at your pharmacy.  Remember to have the following information listed below when you call us for a medication refill: Full name of the person taking the medication Birth date of the person taking the medication Complete medication name Medication dosage to be ordered and frequency Name of the pharmacy refilling the prescription Your telephone number Name and relationship of the person requesting the medication
